- Physician – registration + annual dues = $700
- Non-physician attendees (must be sponsored by a member) = $650

CME Credits
Our program will be eligible for AAFP Prescribed Credit. AAFP Prescribed Credit is accepted by the American Medical Association as equivalent to AMA PRA Category 1 Credit™ toward the AMA Physician’s Recognition Award. When applying for the AMA PRA, Prescribed credit earned must be reported as Prescribed credit, not as Category 1. These CME credits are suitable for physicians of all specialties, not only family physicians.
Saturday afternoon and evening January 26th are set aside for informal and fun activities around town.

The Saguaro is a wonderful hotel at which every day on Jan 24-26, 2019 we will have catered Paleo/ Whole 30-compliant breakfast and lunches, and it will be the location for all of our meetings and movement sessions.
Please be aware that in addition to special concessions for the rooms, the hotel has given PAH concessions for the meeting room in exchange for our members staying in the hotel. This helps to keep the cost for the retreat down, and helps support the organization. If you book outside our block and PAH does not meet the minimum, PAH will have to pay the difference.
If you choose not to stay in the hotel, please add $150 to your fee to cover your breakfasts while at the retreat. Alternatively you can choose not to eat breakfast with the group. Please let us know if this is the case so PAH does not get charged for your breakfast.
The separate retreat registration fee ($700 for PAH attendees, $650 non-physician sponsored attendee, $350 for residents and students) will cover the cost of 6 meals (catered breakfast and lunch for 3 days of the retreat), use of the meeting room, AV equipment, and CME- accreditation. Historically, our retreats had greater than 90% retention rate and we hope you will be able to join us.
ROOM RATES
We have a negotiated room rate of $189/ night for a single or double room, subject to tax.
There will be a one-time porterage fee of $7.00 per person and a daily Housekeeping fee of $3.00 per room. These fees are separate and distinct from the room rate and from taxes.
Our group rates will be honored for PAH attendees three (3) days before group arrival and three (3) days after group departure based on availability.

GROUP CUTOFF DATE
Please book your room prior to January 2, 2019 to take advantage of the negotiated group rate.
ROOM RESERVATION PROCEDURES
To reserve your room, contact the hotel directly at 877-808-2440 and give the name of the Group (Physicians for Ancestral Health) in order to receive your Group’s specially negotiated rates. All individually booked reservations must be guaranteed to the attendee’s major credit card at the time of reservation and the reservation must be received by the Cutoff Date 01-02-19.
GUEST ROOM CHARGES
When reservations are made, Saguaro will require a major credit card to hold the reservation. Each guest will be required to present a valid credit card upon check-in, on which an amount of sufficient pre-authorization can be obtained to cover the room and tax charges and hotel fees for the length of the guest’s stay, plus the anticipated use of the hotel’s ancillary services.
Any changes made to group individual reservations or master rooming lists must be made through the reservations department and submitted in writing 72 hours prior to arrival. Saguaro will do our best to accommodate change requests based on availability at the time the request is made, but cannot guarantee that requests will be accommodated.
